Dear Applicant:

Dear Applicant:

Thank you for contacting Military Missions In Action (MMIA). Our organization is a 501(c) (3) nonprofit dedicated to assisting Veterans with disabilities to achieve independent living. To qualify for assistance in the program, an applicant must meet all of the following minimum conditions:

• Veteran or spouse of veteran with disability

• Needs assistance with handicap accessibility

• Owner and Occupant of the home for ramp build

Enclosed are the Application for Assistance, and Homeowner Agreement. Please complete the forms and return them to MMIA along with the documents highlighted below.  NOTE: processing your application will be DELAYED if any of these documents are missing.

DD-214 (military discharge papers) or copy of VA identification card (front and back)

Deed, lien, or insurance certificate of ownership of the home

We cannot assist Veterans if they have been dishonorably discharged.  We are only completing handicap ramps at this time.  All further home modifications are on hold until further notice and funding.

Once the completed forms are submitted, your application will be reviewed. We reserve the right to request additional information if needed. Please be mindful that there may be a waiting period based on available funding and persons to complete the work.  If all supporting documentation is not turned in, the application process can be delayed. All emergency projects are prioritized based on emergency situations and date application was received.

If you have any questions or concerns, please contact us. Thank you for your service.

Sincerely,

Zak Keisler
